Badge Policy

All LifeScan employees, Contract employees, Temporary employees, and Vendors will be issued a photo identification badge. The badge will have the person’s first name, last name, it will indicate if the person is a Contractor, Temporary, or Vendor, and their company name. Initials, nicknames, titles, or abbreviations will not be put on the badge or associated forms.Electronic Access Control video - starts every 15 seconds

All employees have access to all lobby and employee entrance doors from 5:00 am to 6:00 pm. For access after hours you will need your managers signature. If you need access to a lab, storage area, or other special access (i.e., MERT), you will need that department’s supervisor or manager approval. Security has a list of all personnel with signature authorization.

BADGES MUST BE VISIBLE AT ALL TIMES. CARRYING A BADGE IN A PURSE, WALLET, OR POCKET MAY DAMAGE THE BADGE. THIS WILL RESULT IN BADGE FAILURE!”

LifeScan Security Personnel have been instructed to request to see a LifeScan Identification Badge if it is not worn in plain view.

Color coded badges

LifeScan Photo Identification badges are a combination of identification and an access card. LifeScan has also developed a color coding system to identify individual status:

Blue color stripe and photo border on a badge symbolizes that the person is a LifeScan employee. 
Red color stripe and photo border on a badge symbolizes that the person is a MERT Team member. 
Orange color stripe and photo border on a badge symbolizes that the person is a Temporary employee, this is also used for interns, Co-op’s, and work experience.
Maroon color stripe and photo border on a badge symbolizes that the person is a Contract employee.

How to get a LifeScan Photo ID
Have your Human Resources Representative, Manager, Kelly Services Representative, or their LifeScan employee contact, complete a Badge Request Form.
No appointment necessary. Bring a copy of the Online Badge Request Form or a hardcopy of the form to the Security Operations Center located in Building #2 South Employee Entrance. Access above and beyond “General Access” will require a Manager’s signature.
Complete a Badging Agreement and Non-Employee Security Agreement (if applicable). Refusal to complete agreements will result in Security denying access to the individual.
Sit Down…Smile…You now have a LifeScan Badge. Access will be programmed within 30 minutes.

Forgot or lost your badge ?
If you lose your badge contact Security at extension 6922 immediately so it can be deactivated. If a badge cannot be found within 24 hours a new badge will be issued.
If you forgot or lost your badge you will be issued a Day Use Badge for your convenience. A Temporary Paper Badge is also available.
